Glaucoma is characterized by an increase in intraocular pressure IOP ; . A normal pressure is 15 mm. A single high normal reading 20 to 28 suggests glaucoma. Some patients even get glaucomatous visual field loss with normal eye pressures. The consequence of an elevated pressure is damage to the optic disc and peripheral vision leading to blindness, if untreated. The pressure is elevated when either too much aqueous humor is produced or the outflow channels are blocked. The aqueous humor is produced in the ciliary processes of the posterior chamber and flows around the iris draining through Schlemm's canal located in the angle formed by the iris and cornea. Glaucoma is further classified by the angle of the cornea and the iris. If the angle is greater than 11 degrees and the pressure is elevated, the glaucoma is defined as open-angle glaucoma. Conversely, if the anterior chamber angle is less than 11 degrees and the pressure is elevated, the glaucoma is called narrowangle glaucoma. Approximately 5% of patients diagnosed with glaucoma have narrow-angle glaucoma. The significance of narrow-angle glaucoma is that any drug that dilates the eye may cause the iris to plug the outflow tracts leading to acute angle-closure glaucoma.
