TCO 02 Prehospital Tracheal Intubation In Severely Injured Patients: An Observational Study Christensen Erika F, Hyer CCS Department of Anaesthesiology, University Hospital of Aarhus, Denmark Aims: Advanced prehospital care in Aarhus is by an anaesthesiologist on a mobile emergency care unit MECU ; . The aim was to describe mortality in severely injured patients undergoing out-of-hospital endotracheal intubation with and without anaesthesia. Methods: Severe injury is defined as Injury Severity Score ISS ; 15. Patients with ISS 15 who had been intubated out-of-hospital were included. Results: In 1998-2000 a total of 220 trauma patients had ISS 15 and 172 were brought by the MECU. Prehospital intubation was performed in 74 and among these a total of 62 patients 84% ; received anaesthetics while 12 patients were intubated without prior medication. Half-year survival was 58% among patients given anaesthetics compared to 8% among those intubated without p 0.01 ; . Conclusion: In contrast to other 1 ; , we did not find nondrug assisted prehospital intubation hopeless. Still, the survival was considarably higher among those patients requiring medication before intubation. Teaching prehospital endotracheal intubation and appropriate medication in severely injured to ambulance personell will require considerable resources. In this study we investigated the impact of total cholesterol, lipoprotein a ; Lp a and apolipoproteins a ; Apo a and b ; Apo b on inflammatory markers such as interleukin 6 IL-6 ; , vascular cell adhesion molecule-1 sVCAM-1 ; and intercellular adhesion molecule-1 sICAM-1 and on fibrinogen fib ; in children with FH. Methods: We studied 32 children 16m, 16f ; with FH aged 14.8 1.1 years and 32 healthy control children 18m, 14f ; aged 15.50.7 years. Serum levels of sICAM-1 sVCAM-1 and IL-6, as well as plasma levels of fib were determined using enzyme linked immunosorbent assay ELISA ; . Values normally distributed are presented as meanSE and not normally distributed values as median [IQR]. Pearson's correlation was determined between cholesterol, lipoproteins and inflammatory and haemostatic components. Results: FH children had increased levels of total cholesterol and Lp a ; compared to controls 284.911.3 vs 163.85.2 mg dl, p 0.001 and 9.94[4.53, 29.97] vs 5.45[2.63, 10.67]mg dl, p 0.05 respectively ; rum levels of ICAM-1 and IL6 were also increased in FH children compared to controls 292.112.1 vs 250.114.7 ng mL, p 0.05 and 0.9[0.6, 1.75]vs 0.6[0.4, ml, p 0.01 ; . Lp a ; correlated to IL-6 and fib r 0.39, p 0.001 and r 0.23, p 0.05 respectively ; . Total cholesterol, Apo a ; and Apo b ; correlated with sICAM-1 r 0.23, p 0.05 for all. In addition total cholesterol and Apo b ; correlated with IL-6 r 0.3, p 0.05 and r 0.4, p 0.001 respectively ; . Conclusions: In FH children total cholesterol and lipoprotein parameters were correlated with inflammatory markers and fibrinogen. These findings indicate that inflammatory and haemostatic abnormalities in FH children are related to the underlying disturbed lipid profile and may contribute to the progression of atherosclerosis!

Through the disrupted wound ; . Dehiscence and evisceration may be caused by problems in suturing the wound or by poor tissue integrity. Some of the factors that may predispose a wound to dehiscence are excessive coughing, straining, malnutrition, obesity, and infection. Nursing interventions for the patient who has experienced dehiscence include instructing the patient to remain quiet and to avoid coughing or straining. The patient should be positioned to remove further stress on the wound. If evisceration occurs, the patient is kept on bed rest and the protruding viscera are loosely covered with a warm sterile saline dressing. The surgeon should be notified immediately because treatment consists of reapproximating the wound edges. Chemotherapy has greater response and longer survival rates than radiation. Radiation combined with chemotherapy has been more effective. These treatment modalities are often used with surgery. Nursing Interventions and Patient Teaching The nurse should provide further clarification of the disease and the surgical intervention to the patient and family. The preoperative preparation includes improving the patient's nutritional status by monitoring total parenteral nutrition and providing supplemental diet feedings.
