It is calculated by measuring the distance between the dose that is effective in 50% of patients ED50 ; to the dose that is toxic in 50% of patients TD50 ; . Drugs with a narrow therapeutic index, such as Lithium and Warfarin, must be monitored closely. Drugs with a large therapeutic index are preferred when possible. For the treatment of depression the Selective Serotonin Reuptake Inhibitors SSRI ; are considered first line agents even though all of the antidepressants have been shown to be equally effective. The reason is that the SSRI agents have the largest therapeutic index and have very low toxicity even in overdose. They have a lower risk of serious side effects and death in overdose situations than both the Serotonin Norepinephrine Reuptake Inhibitors SNRI ; and the older Tricyclic Antidepressants TCA ; . A recent Medwatch report 10 25 06 ; from the FDA confirmed these findings. The report stated, ".Venlafaxine Effexor, Effexor XR, SNRI ; over dosage may be associated with an increased risk of fatal outcome compared to that observed with SSRI antidepressant products". Because most of the SSRI's are now generic you will no longer be detailed by the pharmaceutical industry. The SSRI agents should remain your first drugs of choice for the treatment of depression. But some American psychiatrists say the British regulators acted hastily and went too far. "Everybody -- scientists, parents and advocates -- needs an answer to this question, " said Dr. John March, the chief of child and adolescent psychiatry at Duke University Medical Center. But the British regulators, he said, "prematurely closed the story." Dr. March and other experts argue that the cost of leaving depressed children and teenagers untreated is high: depression itself can be a lethal illness. And some psychiatrists say that they have seen "indisputable proof" in their practices that the drugs help seriously ill children. Most of the experts said they would continue to use S.S.R.I.'s to treat children and teenagers. But they added that doctors and parents should closely monitor children for signs of restlessness, agitation, recklessness, unusual behavior or thoughts of suicide, especially during the first weeks of drug treatment and after any increase in dosage. Some anecdotal evidence suggests that suicidal or aggressive behavior, if it is tied to the drugs, occurs within the first weeks after the drug treatment is started. Dr. Graae and others say they start children on very low doses of the medications and that in some cases a low dose is all that is needed. The general view of American doctors is that the benefits of SSRIs far outweigh the risks if the medications are carefully prescribed and monitored.
